Inspector’s narrative

What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 1/21/2026. It indicates that a child was handled roughly by a staff member at the facility. During nap time, a staff member went up to a crying child and used their hands to quickly and forcibly change a child's posture from sitting to lying on their stomach. Facility records were reviewed, pertinent parties were interviewed, a self-report was reviewed from the facility, and video records were reviewed. Based on the information gathered, the following violations have been identified: a staff member handled a child in care in a rough manner. See LIC809D for cited deficiencies of the California Code of Regulations, Title 22, Div. 12. Section 101223 (a)(3). LPA Atherton informed Director Victoria Garcia that this report dated 1/26/2026 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Atherton informed the Director Victoria Garcia to provide a copy of this licensing report dated 1/26/2026 that documents any Type A citation(s) to parents of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Director Victoria Garcia and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to facility representative Victoria Garcia.
