What the inspector wrote
Licensing Program Analyst (LPA) Aman Lama arrived at the facility to conduct a pre-licensing inspection. Present during this inspection was applicant, Angela Bolden. LPA toured the home, inside and out and the following was observed and/or discussed:
During the initial inspection, LPA was having technical difficulties, and exited the home from 9:52am-10:22am to get the technical difficulties resolved.
Normal days and hours of operation: Monday- Friday 6:30am-5:30pm
OFF-LIMIT AREAS INCLUDE: All bedrooms, laundry room, den, dining room, garage and entire backyard.
ON-LIMIT AREAS INCLUDE: Foyer, daycare space, kitchen, bath 2, patio in the outdoor space.
The applicant, Angela Bolden provided proof of control of property. Due to the applicant renting/leasing the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

Smoke detector & carbon monoxide detector were in working order. Fire extinguisher was fully charged.

Weapons are not present according to Title 22. Applicant understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations.

No bodies of water on property at this time. There is a water fountain in the shape of a large vase in the outdoor backyard area, but no water was observed in it. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.

- Applicant understands the responsibilities of being a mandated reporter. Training must be renewed every 2 years.
- Applicant understands the responsibility to know the regulations for anyone providing care
- Inaccessibility of hazards must be constantly reassessed depending on the children in care
- Current facility\342\200\231s phone numbers must be on file with the licensing office at all times
- Baby walkers, bouncy seats, exer-saucers and other similar items are prohibited

Applicant, was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12, or 14 with parent notification.
The California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
A notice of site visit was given to [applicant, licensee or facility representative] and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
