Inspector’s narrative

What the inspector wrote

This is an amended report Continued from LIC 9099 (Page 2) Interviews conducted and records reviewed indicated the facility moved from the previous facility (#364843851) to the current location on or about 12/15/2025. Information obtained indicated the current facility was not operating from 12/19/2025 through 01/04/2026. It was revealed parents and authorized representatives were invited for an open house event at the facility which took place on or about 01/05/2026. Interviews conducted disclosed children are picked up at the front door at the end of the facility\342\200\231s normal operating hours. There were no disclosures made regarding witnessing parents or authorized representatives being denied entry into the home. Although interviews conducted disclosed multiple parents and authorized representatives have not been denied entry into the home, it was disclosed parents and authorized representatives expressed an interest in entering the home prior to the open house event. Information obtained indicated parents and authorized representatives\342\200\231 entry into the home was postponed until the open house event. Records reviewed revealed children were in care from 12/15/2025 through 12/18/2025. Due to conflicting information, it is determined there was not sufficient information evident to support the allegation that Licensee denied a parent access to the day care. Based on observations made at the facility, information obtained during interviews, and records reviewed, it is determined that the allegation could not be substantiated or dismissed. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED. An exit interview was conducted and report was reviewed with the licensee, Diana Rivas. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
