What the inspector wrote
On Monday November 10, 2025, at 9:45 AM, Licensing Program Analyst (LPA) Giovanni Cristales met with licensee Sandra Matheu, who guided the analyst on a facility tour. The purpose of the inspection is to conduct an unannounced Annual/Random Inspection. Licensee is licensed to provide care and supervision for a Large Family Child Care for a capacity of up to 14 children. LPA toured the areas of the home utilized for the Family Child Care to ensure the home follows Community Care Licensing Title 22 Regulations. Present during the time of this inspection is Licensee, two (2) Assistants and four (4) children in care. One of the children is an infant. Family members residing in the home include five (5) adults, licensee, licensee\342\200\231s husband, two son\342\200\231s and one daughter. All adults residing in the home have a Criminal Record Clearance. Days/hours of operation are Monday through Friday, 6:00 AM to 6:00 PM. Incidental Medical Services (IMS) policy was discussed.
Physical Plant:
This is a single story, 4-bedrooms, 4-bathrooms home with kitchen/dining area, living room, laundry room, garage and backyard. There are no pool/spa or bodies of water on the premises. LPA did not observe any pool/spa or bodies of water on the premises. Per licensee, the above ground pool was taken down and removed as soon as Type A citation was issued.
Main care is provided in the living room, and bathroom #1. Children use the bathroom in hallway to the right. The home was inspected inside and out for safety, comfort, cleanliness, telephone service (cell and landline), heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds, medicines (inaccessible in off-limits laundry room and master bedroom with gate barrier). The home is clean and orderly; there is a fireplace in the home (in accessible with metal screen).
LPA observed age-appropriate toys, play equipment, and a TV showing educational videos to the children. All windows are free of cracks, bugs, or debris. LPA Observed one (1) small dog (Maltese). Per licensee all the shot records are up to date, and dog does not interact with children in care. Per licensee when a child becomes ill, the isolation area is in the living room near the front entrance to the left until the parent can pick up the child.
Off limit areas:
Include Bedrooms #1 - #4, bathroom\342\200\231s #2-4, laundry room, garage, and backyard (inaccessible with gate barrier).
Kitchen (Gate Barrier):
Home has a clean and fully stocked refrigerator/freezer.
No chemicals in the kitchen were observed to be accessible. Cleaning supplies are located and made, (inaccessible in laundry room). Hazardous items, sharp knives
(inaccessible in plastic container on top of cabinet above the refrigerator)
that can pose danger to children. Licensee is part of the food program, and provides breakfast, lunch, two snacks, and dinner.
Bathroom
: LPA observed the bathroom and shower/tub to be clean and sanitized. The following are inaccessible: Sharp items, mouthwash, shampoos (high above window), razor, nail polish, etc. LPA observed toilet and faucet to be in operable condition. LPA observed cabinet underneath the sink to be made inaccessible with plastic latch.
Napping:
LPA observed two (2) mobile cribs in the living room, mats inside the closet, enough for children to nap on, and one changing table. Per the licensee, infants are monitored every 15 minutes and documented. LPA observed the sleep logs to be complete and current.
Outdoor :
Backyard currently inaccessible due to plumbing repair (inaccessible with latch on sliding door and gate barrier in kitchen). LPA observed the backyard to be clean of debris and age-appropriate swing set playing structure (anchored to floor). LPA observed cement, dirt and grass, flooring for proper playing. The backyard is completely fenced all around (wooden picket fence).
The AC/Heating unit is located on the right side of the home and is blocked by barrels, making it inaccessible to children.
Advisory/Other:
First Aid kit was observed to be full of supplies readily available (scissors/ tweezers), inside closet upon home entrance to the left. CPR/First Aid, Expires:

Electrical outlets (are inaccessible with plastic covers), no recalled and prohibited toys or sleep/ play equipment were observed on the premises. LPA did not observe any window cords (wooden shutters). Roster complete and maintained current.
The smoke and carbon monoxide detectors are in operable condition. LPA observed the required fire extinguisher in the kitchen to the right of the sliding door (2A10BC) being fully charged with needle in the green (last served on
6/19/2025
).
Per licensee there are no weapons/firearms in the home. Per licensee nobody smokes in the home. LPA observed documents to be posted in a prominent, publicly accessible area (License, PUB 394, Earthquake Preparedness, facility sketch, Emergency Disaster Plan and verification of fire and earthquake drills,

) to be complete and current. Per licensee, transportation is not being provided at this time.

LPA discussed the safe sleep regulations with the licensee and also instructed the Licensee to visit the Child Care Licensing Safe Sleep webpage at

as an additional resource.
LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following was discussed with the Licensee:
Mandatory licensing forms for the children\342\200\231s files, facility forms/records, and information to be posted in the family child care home; Requirements to conduct fire and disaster drills once every six months and record it; Role and responsibilities of being a mandated reporter (

) were reviewed;
Licensee was reminded that 100% supervision is required at all times to children in care; Licensee were made aware that it is her/their responsibility to know the regulations as well as anyone who assists in providing care; Licensing must have the facility\342\200\231s phone number at all times; if the phone number is changed, licensing must be notified; Regulation prohibits the smoking of any kind during the operation of the day care.
Licensee was advised of the requirement to report Unusual Incidents. A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. Licensee was informed to utilize the

Licensee advised of the Notice of Site Visit, which must be posted at the entrance of the facility for a period of 30 days whenever a licensing inspection is conducted. If a Type A deficiency is cited, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224. If these requirements are not met, civil penalties per violation will be assessed.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, inspect the facility. The Licensee shall permit the Department to inspect the family childcare home and to privately interview children or staff, to determine compliance with or to prevent violations of family childcare laws or regulations, also enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
Prior to making alterations or additions to a family childcare home or grounds, the licensee shall notify the Department of the proposed changes, including, but not limited to, the following: Conversion of a garage (either attached or detached) into a "childcare" room; Room additions to the family childcare home.
Any change from an area of the family childcare home previously identified as "off limits" to an area where care and supervision will be provided to children in care. The licensee shall provide the Department with a copy of an inspection report when an inspection is required by the local building inspector as a result of the alteration, addition or construction.
Notice of Site Visit: A notice of site visit was given and must remain posted for 30 days.
