What the inspector wrote

On Monday, June 2, 2025, Licensing Program Analysts (LPAs) Hanna Cha and Sherell Braddock met with facility assistant, Liliana Costa. LPA’s contacted licensee via phone who informed LPA’s that she was away at court and the visit can be conducted with the assistant. The purpose of the inspection was to conduct an unannounced case management inspection fora self-reported Unusual Incident Report (UIR) received at Palmdale Regional Office on 4/28/25,5/29/25 and 5/30/25. LPAs disclosed the purpose of the inspection to the facility assistant. When LPAs arrived at the facility there were 0 children with 2 staff. During the visit 1 child arrived. The days and hours of operation for the program is Sunday through Saturday from 23 hours a day. LPA reminded the assistant that staff must remain awake with children during overnight care. During the inspection, LPAs received a copy of the roster, reviewed staff files, interviewed staff, and toured the inside and outside of the facility. LPAs completed a safety inspection of the facility at approximately 12:55 No safety concerns observed at this time. Further investigation is needed regarding UIR. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and the report was reviewed with the facility assistant Liliana Costa.
