What the inspector wrote

Based on the information obtained from the interviews conducted, the above allegations have been substantiated based on a preponderance of evidence. An immediate civil penalty in the amount of $500 dollars was assessed as of result of the absence of supervision. The facility was cited Type A deficiencies as of result of the violations in accordance with Title 22 of the California Code of Regulations. Facility was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. A copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children currently and newly enrolled at the facility during the next 12 months and facility must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian and place it in each child's file. If these requirements are not met, civil penalties per violation will be assessed. An exit interview was conducted, a copy of this report, a notice of site visit and appeal rights were provided. ** This report has been amended to provide additional information on Page LIC9099-D

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time. This requirement is not met as evidenced by: Based on the investigation, it was determined that on or about 9/29/22, during nap time, there was lack of visual supervision of children resulting in the inappropriate contact between child #1 and child #2. This is a Type A violation which poses an immediate health, safety or personal rights risk to persons in care.
