THIS REPORT IS AMENDED FROM THE ORIGINAL REPORT ON 9/28/2023. On 9/28/2023, Licensing Program Analyst (LPA) Calloway made an unannounced Case Management inspection at the above facility. LPA met with Assistant Director who granted access. Upon initial walk through, LPA observed 23 preschool children in active play outside and seven (7 infants) in active play and no school aged children. While in the center thirty nine (39) school aged children arrived on the bus. LPA did a second walk through and observed twelve (12) napping preschool children and twelve (12) preschool children in active play outside and seven (7) infants: five in active play, two napping and eight staff. The amount of children in census has placed the center over the capacity limits for the required licenses. This is a combination center with 367750032 and 367750033. Per Title 22 Regulations, Division 12, Chapter 1, of CCR, there is a Type A deficiency cited: 101161(a) for Limitation on Capacity and an Immediate Repeat Violation Civil Penalty for $1,000.00 were cited during this inspection. See 809-D page and LIC 421IM page for deficiencies. Upon receipt of a Type A deficiency licensee shall post the report for 30 days in addition to the Notice of Site Visit and provide copies of the licensing report to parents/guardians of children in care at the facility. This report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months and licensee will obtain a signed acknowledgment of Licensing Reports (LIC 9224) from parent/guardian and place it in each child's file. If these requirements are not met, civil penalties will be assessed. An exit interview was conducted a copy of this report was read, discussed, and provided to Sonia Galvaz, Assistant Director, along with a Notice of Site Visit, and Appeal Rights. The Notice of Site Visit must remain posted for thirty (30) consecutive days, failure to maintain posting will result in a $100 Civil Penalty per day.
