What the inspector wrote

On June 3, 2025, at 08:10am, Licensing Program Analyst (LPA) Zirbes met with the facility representative Jamie Jarrett for the purpose of conducting an unannounced Plan Of Correction (POC) inspection regarding the deficiencies issued on May 21, 2025. Upon LPA arrival, there were 16 school age children and two staff members, (staff 1 (S1) and staff 2 (S2)) providing supervision. When LPA arrived on site, LPA observed S1 was outside on the playground with 11 school age children and S2 was inside the classroom with six school age children. Per conversation with S1, S1 is the fully qualified teacher and S2 is an aide. S1 was approximately 20 feet away from the classroom door. S1 expressed S1 had stepped away from the classroom to step into the sun with the classrooms pet, a bearded dragon. After LPA arrival, LPA observed S1 station herself in the doorway of the classroom allowing S1 to supervise the children who were outside and maintain supervision of S2 and the children inside. When asked for clarification, regarding LPAs observation upon arrival, S1 expressed S1 had been away from the classroom for less than three minutes when LPA arrived on site. LPA and S1 discussed the supervision requirements. During this inspection, LPA completed a tour of the classroom and observed the first aid supplied labeled keep out of reach of children are stored in a locked cabinet. Per record review, a disaster drill was completed, and the Center completed the LIC 9224. Based on LPA observation and record review, three of the four deficiencies cited on May 21, 2025, have been cleared. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided. Exit interview conducted and report was reviewed with the facility representative Jamie Jarrett.
