What the inspector wrote

Licensing Program Analyst (LPA) Maddox met with Saleena Smith, Applicant, Applicants minor daughter, and Mariana, Applicants Sister for the purpose of conducting a Pre licensing inspection for a Pre-school (PS) Child Care Center. This facility is located in a business center located at the above address in Suites G & H. Present during this inspection were Saleena Smith, Hrs. of Operation: Mon - Fri from 2:00 pm to 6:00 pm. Indoor Space : the School age room is located immediately down the hall as you enter Suite G (one room on the right - the Learning Center, and a second room on the left "game room", There is a large room located off the game room that applicant is requesting to use as an indoor/outdoor area, this area's floor is actually flooring used for dance, gymnastics, tumbling and measured at: 34 X 26 = 884/75 = 12C (for outdoor space). 884/35 = 25 (for indoor space). Game Room: 23 X 7 = 161/35 = 5 Learning Center: 22 X 30 = 660/35 = 19 All disinfectants, cleaning solutions and other items that are dangerous to children were found to be inaccessible to children (in the office and the attic). Furniture and equipment were inspected for age appropriateness and are in good repair. Telephone service was verified and operational. Heating, lighting, and ventilation are adequate. LPA observed age appropriate toys and materials. Outdoor Play Yard: The outside play yard was measured today to establish how many children can occupy the yard at one time. During this inspection, applicant informed LPA that there is a designated play yard for the children that's located across the parking lot to the left (facing the building). This area was surrounded with chain link fencing, there was no outside play equipment present during this inspection. Applicant states the owner will not allow her to place any equipment or build anything in the fenced area (located in a dirt area) until she has an approved license from the Department. Applicant states the Owner is also allowing her to use some of the parking spaces/lot to use for children as well whenever she needs to use the space. Applicant states she has 4 tents and cones she can place on the yard while children are present. Measurements were as follows: had no outside equipment 40 X 40 = 1,600/75 = 22C This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Medications are kept in a safe condition, and free of hazards. A review of medication policy indicates that prescription and non-prescription medication is administered only with parent's written permission. **Children will be Signed in and out manually. The parent board is located in the lobby area as you enter the building. LPA printed out form LIC 311 and informed applicant of the required forms that must be posted and kept in files for children and staff. Also, the requirement to document Fire/earthquake drills. was reviewed and has all the required forms posted. LPA has verified applicant has current CPR/First Aid training (exp 6/2024), current Mandated Reporter Training, and required immunization's. The SA bathroom is located in the hallway area by the office, the bathroom has 1 toilet and 1 sink, the staff bathroom is located in the same hallway with 1 toilet and 1 sink as well. The toilets and sinks are functioning properly and are age appropriate. All toilets and hand washing facilities are in safe and sanitary condition. LPA observed soap, toilet paper and paper towels readily available, hot water is the appropriate temperature. Food Service : Applicant states she will supply breakfast, lunch, and snacks. LPA informed Applicant to check with CCRC for a Food Program. There is an area that will used as the food preparation area, this is not a full kitchen but has a refrigerator, microwave, snacks, water. All chemicals in the kitchen were kept separate from the food. All cleaning solutions/chemicals and compounds in the kitchen were locked by key and had child proof latches. Food that was stored in containers was labeled properly. The facility has ensured and establish measures to keep the facility free of flies and, insects and rodents. LPA observed that this facility has an operating telephone 4 fully charged fire extinguishers (2A10BC). Applicant was not sure if the carbon monoxide is connected to the hardwired smoke detectors. LPA requested Applicant purchase an independent carbon monoxide detector just in case. All storage containers for solid waste, including moveable bins, have tight fitted covers that are kept on, and are in good repair. Indoor Measurements: Learning Center (22 X 30 = 660/35 = 19); Game Room (23 X 7 = 161/35 = 5) Indoor/ outdoor room (34 X 26 = 884/35 = 25; 884/75 = 12). Outdoor Measurements: 40 X 40 = 1,600/75 = 22 Bathroom: 15 (another bathroom is located on the other side of the office; this bathroom has 1 toilet and 1 sink as well which could give a capacity of 30). Fire Clearance has been received for the requested capacity of 25 School Age children. Exit interview conducted, a copy of this report discussed and left at facility.
