What the inspector wrote

On November 8, 2024, at 09:15am Licensing Program Analyst (LPA) Zirbes met with facility representative Casie Gilham and Administrator Lisa Burtner to conduct an unannounced case management inspection. The purpose of the case management was to follow up on a self-reported unusual incident report (UIR) submitted to the Department on October 28, 2024. The unusual incident report was regarding a potential personal rights incident involving child 1 (C1). Upon LPAs arrival, there were six infants in care with one fully qualified teacher and one aide providing supervision. During this inspection, interviews were conducted with staff. Child interviews were not completed due to the age and development of the children in the infant classroom. In addition, LPA reviewed staff and child files. Furthermore, LPA completed a safety inspection of the Center at 09:30am, no deficiencies were observed during the safety inspection. Further investigation is required in order to resolve the case management. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Administrator Lisa Burtner.
