On October 7, 2025, at 2:00pm, Licensing Program Analyst (LPA) Zirbes met with the Facility Representative, Mimi Simmons. The purpose of the inspection was to conduct an unannounced annual/random inspection. During the inspection, LPA toured and inspected the Infant program in accordance with the facility sketch. LPA observed a total of four infants, with one fully qualified teacher providing supervision. The infant area consists of one room and an outdoor activity space. The center also is licensed to operate a Preschool Program (#367750054). The center operates 7:00am -6:00pm, Monday through Friday. Indoor/Children's Area: Indoor activity space for infants is physically separate from space used by children in the preschool component. Infant center is equipped with appropriate furniture and equipment, changing table and feeding table. Furniture is maintained in good repair and safe condition (age appropriate). LPA did not observe sharp points or edges, or small parts that could be swallowed. Per staff interviews equipment, toys and furniture are sanitized daily. There is one hand washing sink within arms length of the changing table. No infant is left without supervision (visual), nor left unattended. At the time of this inspection, the facility was in compliance with the staff/infant ratio and was not exceeding the approved capacity for the facility. Furniture and equipment were inspected for age appropriateness and good repair. Based on LPAs observations the toys and equipment were in good repair at the time of this inspection. There is a Infant Daily Report which documents the times the infant napped, time of diaper changing and what the infant ate. Report is completed each day by the staff. Used diapers are put in a trash can in a cabinet which is stored near the changing area. Each infant has container labeled with their name for the storage of personal items. Report continued on page two Report continued from page one LPA observed the cleaning compounds were stored on a high inaccessible shelf in the classroom. The infant center does not have a fire place. The building has central heating. The building does not have air conditioning however there are ceiling fans are in the room. The room temperature was comfortable at the time of this inspection. LPA observed combination carbon monoxide and smoke detectors in the center along with Fire Extinguishers. There is a working telephone on the premises. There is an isolation room for ill children. The facility maintains electronic sign in and out records. Based on LPA review the sign in/out sheets were current and accurate. No smoking is allowed on the premises. LPA did not observe baby walkers or other prohibited play equipment on the premises. Napping : LPA observed cribs and cots. LPA observed the crib mattresses were made of a cleanable vinyl material and were in good repair. The indoor activity space and sleeping space was physically separated by a barrier 19 - 30 inches in height. Therefore the separation of the spaces was not accomplished as specified in (b), which requires the barrier to be four feet high. When the Administrator was questioned about the barrier the Administrator reported the Center was licensed with this same barrier in place. A type B citation was issued refer to LIC 809D. Sheets are cleaned and changed everyday or more often as necessary. Other: Electrical outlets are inaccessible, there are no recalled and or prohibited toys or sleep/play equipment observed on the premises. Restrooms: At this time all infants are in diapers the infant area does not have a bathroom. A diaper changing area was located in the infant sleeping area. The area was clean and sanitary during this inspection. Outdoor: Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. The outdoor play area was completely enclosed by fence. LPA observed a small climbing structure placed on sand. The climbing structure was secured to the ground. An umbrella was present to provide a shaded rest area. The teachers take water and cups to the outdoor play space for the infants. Report continued on page three Report continued from page two I nfant File and Staff File Review: LPA reviewed a total of five infant files and two staff files. Three of the five October 2025 modifications to the Infant needs and service plans were not signed by the authorized representative (Child 1, Child 2 and child 3). A verbal advisory was provided. When LPA arrived to the Center, Staff 1 was alone at the Center and proving supervision to four infants, staff 2 arrived on site at approximately 2:00pm. S1 left the Center leaving S2 alone at the Center and providing supervision to four infants. Staff files were reviewed during the inspection. Per LPA review, S1 did not have a pediatric cardiopulmonary resuscitation (CPR) and pediatric first aid training within the staff file. S2 CPR first aid expired 6/2025. Administrator confirmed neither S1 nor S2 had current CPR/First aid training. Administrator did not know why or how the training was not completed by S1 or S2. Therefore based on record review and interviews, at least one staff member who is trained in CPR/First aid was not on site from 1:30pm - 2:30pm. A type B citation was issued Facility Records Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Postings: The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster, car seat law (PUB 269), emergency disaster plan. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Report continued on page four Report continued from page four Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/ safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
