What the inspector wrote

On October 7, 2025, at 01:30 p.m., Licensing Program Analyst (LPA) Kendal Zirbes conducted an unannounced Plan of Correction (POC) inspection and met with Facility Representative Mimi Simmons. LPA disclosed the purpose of the inspection and was granted entry into the facility by the facility representative. LPA completed a tour of the Center and observed the following: 1. At the time of the inspection there were 4 infants in care and one fully qualified teacher providing supervision. 2. Per the posted emergency disaster record a fire drill was completed on 10/1/25. Prior to this inspection LPA received a copy of the staff training. Based on LPAs observation, the citations issued on September 24, 2025 were corrected. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Lisa Burtner.

    Based on observation, interview, and record review, the licensee did not comply with the section cited above when a zero staff members were on site with current CPR/First Aid training, which poses/posed a potential health, safety or personal rights risk to persons in care.
