What the inspector wrote

On 04/21/2026, Licensing Program Analyst (LPA) Hanna Cha conducted an unannounced case management visit to follow-up on an Unusual Incident Report (UIR) received at Palmdale Regional Office (PRO) on 04/03/2026. LPA observed one teacher providing care to two infants, two staff providing care to seven toddlers, and two staff providing care to 10 preschool-age children. Description of Incident: Parent of Child #1 (C1) claimed that staff stole C1 “chain” (necklace). The facility denied parent’s accusation. The facility reported to have a video recording of C1 leaving the facility with the necklace around his neck. During the visit, LPA acquired a copy of the roster, reviewed files, and interviewed staff and parents. Based on staff and parent interviews, C1 attended the facility a total of three days. C1 last day at the facility was 04/01/2026. LPA reviewed the video recording dated 04/01/2026 of C1 leaving the facility with a necklace on and reviewed C1 sign-in and out sheets. Per staff and parent interviews, inconsistent disclosures were made regarding if the necklace was stolen or switched out with a different necklace. LPA determined further follow-up is needed. A Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted and report reviewed with the Director, Tiffany Boyce.
