Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Maddox met with Applicant, Dr Mohammed and Director Tasha Owens-Green today for the purpose of conducting a second Pre-licensing inspection. During the initial inspection, the outdoor play yard wasn't set up, so LPA measured the entire yard in it's entirety. Since the initial inspection, Applicant has installed artificial grass, rubber chips, toys, and fencing. This led to a second Pre-licensing inspection to re-measure the individual sections of the play yard. Measurements were as follows: Yard #1(Todd/PS) - located immediately as you enter the play yard 322 sq ft Yard #2(Todd/PS) - middle of the play yard 1840 sq ft Yard #3(Todd/PS) - located to the right side of SA play yard 704 sq ft Yard #4 (SA) - Rear of the play yard 624 sq ft Preschool yard totals 2,866 sq ft and can accommodate a capacity of 38 School-age yard totals 624 sq ft and can accommodate a capacity of 8 Waiver needed to allow Toddlers and Preschool children to share the play yard with alternating schedules, a waiver is not needed for the School-age, yard space is enough to accommodate requested capacity. Applicant is requesting to use classroom #3 for Preschool children (Early Head Start) until 3:00pm daily, after 3:00pm they want to use this classroom for School-age children (Applicant states they will swap out classroom furnishings for the School-age children), waiver required for this request. In addition, if the waiver is approved, the capacity for Preschool children can not exceed 15 children after 3:00pm due to the bathrooms. There are 2 bathrooms, each with 1 toilet and 1 sink, once School-age children are present, they will require their own bathroom. Final file review will be conducted by LPM, waivers need to be approved by Regional Manager before licensure.
