What the inspector wrote

On August 29, 2025, at 11:40 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual inspection. LPA met with Facility Representative, Lynn Leavitt and disclosed the purpose of inspection. LPA were led on a tour of the inside and outside the facility by Facility Representative. There were three staff members and no children during this inspection due to the facility being closed for a teacher preparation day. This facility serves children ages 2-5 in Classroom 303, 304, 404B and the fellowship hall and were inspected. Classrooms 306, 309 ,402, 403 are not currently in operation due to low enrollment. This is a full day program which operates year round schedule. Days and hours of operation: Monday- Friday from 7:30 a.m. to 5:30 pm. All toilets and hand washing facilities were observed to be in safe and sanitary operating condition. The floors in the facility were observed to clean and safe. Solid waste storage containers were observed to have tight-fitting covers and in good repair in all the classrooms. Drinking water is available both indoors and outdoors. The facility provides AM and PM snack. Children bring their own lunches. Children bring their own water bottles and filter drinking water is available at the facility. Menus are posted weekly. LPA observed areas around high climbing equipment have sand to absorb falls. LPA observed the play equipment is age appropriate. Shade was observed in the outdoor activity space being provided by a canopy and umbrellas. The facility has a functioning carbon monoxide detector and a first aid kit that meet statutory requirements. The last fire drill was conducted and documented on 05/13/2025. Earthquake drill was conducted and documented on 4/15/2025. Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated there are no firearms or ammunition allowed or stored on the premises. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets. Facility utilizes a physical sign in/ out procedure. Facility representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. No Deficiencies cited during this inspection. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Lynn Leavitt and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Facility Representative, Lynn Leavitt.
