Based on the information gathered during the investigation, there is not enough corroboration and evidence regarding the allegation. Due to conflicting information obtained from the interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the above allegation is found to be unsubstantiated. Exit interview was conducted with Director, Nancy Betancourt and a copy of this report and Appeal Rights were provided. A notice of site visit was given and must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
