What the inspector wrote

On 06/27/2025 at 10:15 AM Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced case management inspection regarding a self - reported incident. LPA met with Director Elizabeth Cortese. LPA discussed the purpose of the inspection and was led on a tour of the facility. There were 58 children with 14 staff present at the time of this inspection. On 06/09/2025, the director self-reported an incident involving Child #1 (C1). Per Director, the incident occurred on 06/02/2025 at approximately 10:50am. Director reported that C1 was on the playground with 14 children supervised by 2 staff. Staff 1 (S1) was assisting C1 with working out a problem with another friend. S1 was guiding C1 by the hand to return a toy to a friend when C1 dropped their body and S1 held on to their hand to assist C1 to the ground. As C1 stood up S1 noticed C1 cradling their right arm. C1's parents were notified of the injury and C1 was picked up at 2:30 pm. Director was notified that C1 received medical treatment for the injury the same day. Interviews were conducted with the the Director, Staff #1 (S1) and Child #1 (C1's) Authorized Representative. This injury was determined to be an accidental injury. No deficiencies cited. An exit interview was conducted with Director, Elizabeth Cortese, and a copy of this report, and Appeal Rights were provided to the director. Notice of Site Visit was provided and is required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the bulletin board at the entrance.
