On 10/08/2025 at 9:45 AM, Licensing Program Analysts (LPAs) J.C. Valdez and Jody Dye , made an unannounced visit to follow up on a self-reported incident that occurred on 09/22/2025, wherein C1 fell and dislocated their elbow. LPAs met with the Director Michelle Zamora and assistant Director Sara Sanders. LPAs identified themselves and disclosed the purpose of the visit. LPAs were provided a copy of the LIC 500 for review. LPAs toured the facility, conducted interviews, and inspected the area where the incident occurred. LPAs took pictures of the equipment involved in the incident and deemed it to be age appropriate. After investigating the incident that occurred on 9/22/2025 LPAs determined that the facility was not at fault. The facility has adequate staff and supervision, infrastructure, and policy and procedures to ensure and environment safe for the care and supervision of children. An exit interview was conducted, and the report was reviewed with the Director Michelle Zamor assistant Director Sara Sanders. A Notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. No deficiencies were cited.
