What the inspector wrote

On 12/03/2025 at 10:30AM Licensing Program Analyst (LPA) Dana Stevens, conducted an unannounced Annual Inspection and met with Director, Marcela Zavala.  LPA disclosed the purpose of the inspection and toured the facility. The facility provides care for children ages 2 - 5 in classrooms Preschool 1, Preschool 2, Pre-K and 2's. Hours of operation are Monday - Friday 6:00 AM - 6:00 PM. At the time of this inspection the following ratios were observed: 2's classroom: 13 children present with 2 staff Preschool A: 15 children present with 2 staff Preschool B: 14 children with 2 staff Pre-K: 12 children with 1 staff There were no bodies of water observed within the premises. Director stated there are no firearms/weapons on the grounds. Furniture and playground equipment are kept in good condition. Facility has a functioning carbon monoxide detector that meets requirements. All toilets and hand washing facilities are safe and working in sanitary conditions. The facility provides breakfast, lunch, and snack. Children bring their own water bottles and filter drinking water is available at the facility. Menus are posted monthly. Facility's kitchen and food storage areas are clean and sanitary. All food is protected against contamination and any contaminated food is discarded immediately. Outdoor activity space surface is maintained in a safe condition and free of hazards.  All storage containers for solid waste have a tight-fitting covers that are kept on and in good repair. The child care center is clean, safe, sanitary, and in good repair at all times to ensure the safety and well-being of children. Roster and children's files were reviewed and found complete. Staff files were reviewed and four (4) out seven (7) files were found to be incomplete. Type B citation issued on the attached LIC809D. The last fire/disaster was conducted and documented on 12/03/2025. Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than twelve (12) children in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets. Facility utilizes an electronic sign in/sign out procedure. Director was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Deficiencies cited on the attached LIC809D. Exit interview conducted and report was reviewed with the facility representative, Marcela Zavala. A notice of site visit was given and must remain posted for 30 days.
