What the inspector wrote

On 4/6/22 at approximately 1:35 PM, Licensing Program Analysts (LPAs) Saraliz Velando and Selina Siao conducted an unannounced case management inspection. LPAs were met at the entry point by Assistant Director, Leena Saeed. LPAs introduced themselves, displayed their department identification and explained the purpose of the visit. The purpose of this visit was to review California Department of Public Health (CDPH) Guidelines regarding Covid-19. LPAs were provided a tour of the facility by the Assistant Director. LPAs observed the facility was within ratio & capacity. The classrooms were toured to ensure adequate social distancing and hand-washing stations. LPAs observed sufficient and current resources on hand. Covid-19-related safety and informational posters are visibly posted for parents, children, and staff. Per the assistant director, facility staff, parents and children are routinely reminded of proper procedures to ensure safety. Following the tour, LPAs discussed with Assistant Director, Leena Saeed, current COVID-19 guidance including but not limited to masking, vaccines, food service, visitors, isolation for illness, and responding to Positives and Outbreaks. Assistant Director is aware that mask use is strongly recommended. LPAs provided the assistant director with COVID-19 printouts of CDPH guidance and the weblink to the current guidance https://www.cdph.ca.gov/Programs/CID/DCDC/Pages/COVID-19/Child-Care-Guidance.aspx ). LPAs also discussed and provided the Assistant Director with printouts of Provider Information Notices (PIN) 22-08-CCP, 22-01.1-CCLD, and 22-10-CCP related to current COVID-19 guidance. When asked, the Assistant Director outlined the COVID-19 protocols that have been implemented at the center to include but not limited to; temperature checks at drop-off, check parents and children temperatures, staff, children and parents continue to wear masks, children wash their hands as soon as they enter classroom. Parents do not enter the classroom. Exit interview conducted and report was reviewed with the Assistant Director, Leena Saeed. A notice of site visit was given and must remain posted for 30 days.

    Personal Rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule,coercion, threat, mental abuse or other actions of a punitive nature including but not limited to interference with functions of daily living including eating, sleeping ... to physical functioning. This requirement was not met as evidence by: Based on information obtained from the facility, records reviewed and interviews conducted, a staff person violated the Personal Rights of a child by placing her foot on a child’s neck on 2/17/22. This poses a potential health and safety risk for children in care.
