Incident investigation·April 28, 2026

On 4/28/26 at 12:15 pm, Licensing Program Analysts (LPAs) Keely Messerschmidt and Jody Dye arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on 4/14/26, per Director. LPAs met with Director Gloria Sanchez and provided purpose of inspection. At the time of inspection, LPAs toured the facility, took census, interviewed and reviewed documents previously submitted to the department with Director. At this time LPAs will return for further investigation. An exit interview was conducted, and a copy of this report was provided. Notice of site visit was provided and must remain posted for 30 days.
