What the inspector wrote

**This is an amended version of original report dated 02/20/2026** Per California Code of Regulations, (Title 22, division 12 & Chapter 1) one (1) Type B citation is being cited on the attached LIC 9099-D. An exit interview was conducted with Assistant Director, Natahsa Aldaco, and a copy of this report along with the Appeals Rights (LIC 9058) was provided. Notice of Site Visit must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

This requirment was not met as evidenced by, based on interviews and record review, facility did not met this reqirement when zero (0) bottle feedings were recorded for Child 1 (C1) on 12/04/2025, which posed a potential health, safety and personal rights risk to children in care.
