This is an amended report that was created on 08/29/2025. On August 29, 2025 at 11:40 AM, Licensing Program Analyst (LPA) Gloria Gonzalez conducted an unannounced Case Management inspection to follow-up on a self-reported incident that occurred on or about 01/15/2025. LPA disclosed the purpose of the inspection and was granted a tour into the facility by Licensee representative, Julie Pretzer. There were thirty-four (34) children six (6) staff members at the time of the inspection. Investigator Thomas Smith, with the Department's Investigations Branch, investigated the allegation that a staff member (S1) made inappropriate contact with a child in care (C1) on or about 01/15/2025. During the course of the investigation, interviews were conducted with S1, C1, and other pertinent parties. These interviews were conducted by the Department and the San Diego Police Department. Based on the investigation by the San Diego Police Department, S1 was arrested for the conduct. The preponderance of the evidence standard has been met to support a personal rights violation against C1. LPA, Gloria Gonzalez informed Licensee representative that this report dated 08/29/2025 documents a Type A citation which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care. ***No amendments to this page. Also, LPA, Gloria Gonzalez informed the Licensee representative to provide a copy of this licensing report dated, 08/29/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. On this date LPA reviewed the inspection authority to interview children was discussed with Licensee representative. A copy of this report and a Notice of Site Visit (LIC 9213) was given to Licensee representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. An exit interview was conducted and the report was reviewed with Licensee representative.
