On October 27, 2022, at 9:59 AM, Licensing Program Analyst (LPA), Marie Hernandez, conducted an unannounced Case Management Inspection to deliver the conclusion of the incident review of 06/14/2022. The facility self reported the incident to the Department pertaining to child in question. LPA attempted to conduct the case management inspection but the facility was closed due to summer break of children and reopened on 08/31/2022. On 09/13/2022, LPA conducted the case management inspection of incident but the incident required further review at that time. During today’s case management inspection, LPA met with the Facility Director, Rowena Ohly. Present during today's inspection were twenty children with eight staff. Through the course of the case management inspection, LPA toured the facility and conducted confidential interviews with the staff and the Director. The child in question could not be interviewed as child aged out of the program and no longer attends the facility. The parent of child and the former staff person in question could not be interviewed as they did not respond to any of LPA’s calls. The staff person in question is no longer employed at the facility. The facility reported that on 06/10/2022 at 2:30 PM, the staff person in question allegedly pushed and yelled at child in question. However, the child did not sustain any injuries or bruising from the incident. This concludes the incident review due to insufficient evidence and unable to interview the child in question, parent, and staff person in question. An exit interview was conducted and the report was provided to the Facility Director, Rowena Ohly. The Notice of Site Visit was provided and was posted by the Facility Director .
