On October 1, 2025, at 10:30 AM, Licensing Program Analyst (LPA), Dana Stevens, conducted an unannounced Case Management Inspection to follow up on two self-reported Unusual Incidents. LPA met with the Director, Rowena Ohly and informed her of the reason for the visit. There were 42 children present with 6 staff during this inspection. On 09/18/2025, the Department received two Unusual Incident Reports (LIC624) from the facility. The first incident occurred on 09/15/2025 and involved Child 1 (C1) and the second incident occurred on 09/17/2025 and involved Child 2 (C2). During today's case management inspection, LPA toured facility and conducted confidential interviews with the Director and the staff involved. These incidents require further review. An exit interview was conducted and copy of this report was provided to Director, Rowena Ohly. Notice of Site Visit must be posted for thirty days.
