On October 23, 2025, at 01:40 pm, Licensing Program Analyst (LPA), Dana Stevens, conducted an unannounced Case Management Inspection to deliver the conclusion of the review of two self reported Unusual Incidents. LPA met with facility representative, Gloria Carreto, and informed her of the reason for the visit. There were 30 children present with 9 staff during this inspection. On 09/18/2025, the Department received two Unusual Incident Reports (LIC624) from the facility. The first incident occurred on 09/15/2025 and involved Child 1 (C1) and the second incident occurred on 09/17/2025 and involved Child 2 (C2). During the Unusual Incidents review, LPA toured facility and conducted confidential interviews with the Director, staff and daycare parents and reviewed facility video footage. Based on information obtained in interviews and video review, no violations of child care regulations occurred. No deficiencies cited. An exit interview was conducted and copy of this report was provided to facility representative. Notice of Site Visit must be posted for thirty days.
