What the inspector wrote

On September 11, at 9:15 a.m., Licensing Program Analyst (LPA), Angela Nguyen conducted an unannounced Annual/ Random Inspection with Facility Representative, Ana Valadez and disclosed the purpose of the inspection and toured the indoors and outdoors of the facility. Classroom #1 has 9 children being supervised by 2 teachers. The facility operates Monday- Friday from 9:00 a.m.- 12:30 p.m. Appropriate Title 5 staffing ratios are maintained during the inspection. This program operates following the yearly schedule of Mountain Empire Unified School District. Furniture and equipment are in good condition. Floors in the classroom is clean and safe. Toilets and hand-washing equipment are in safe and sanitary operating condition. Paper towels and toilet paper are available. The bathroom is lighted and has ventilation. Disinfectants, cleaning solutions, and other hazardous items were made inaccessible to children in locked cabinets during time of inspection. The facility provides lunch to the children. Food is prepared and delivered to the program from the main kitchen at the elementary school. Menus are posted monthly. Classrooms have a carbon monoxide detector that meet requirements. The last fire and disaster drill were conducted and documented on 09/10/2025. Facility Representative stated that there are no swimming pools or other bodies of water, firearms or ammunition allowed or stored on the premises. Drinking water is available in the classrooms via water bottles and filtered water dispenser and children bring their individual refillable water bottles outdoors. Additional water bottles are available during outdoor activities. The playground is fully fenced and has age appropriate play equipment for the children. Shade is provided by a pop up canopy. A new shade will be installed in a week per Director Montano. Rubber cushioning is surrounding the play structure to damped child's falls. LPA observed 2 holes approximately 1 foot in length in the rubber flooring under the slide and by the stairs at the play structure. Facility Representative states that a work order has been submitted on 8/13/2025 to repair the rubber cushioning. Facility covered th hole with a rubber mat during the inspection. Facility Representative stated that all employee\342\200\231s working at this state preschool have received criminal record clearance with the Mountain Empire Unified School District. Facility Representative stated that all employees working at this preschool have obtained a health screening and TB as a condition of employment with the Mountain Empire Unified School District Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than eight (8) children in care. Facility Representative CPR and Pediatric First Aid expire 8/2027. The name of the facility representative and the fully qualified teacher designated to act in the facility representative\342\200\231s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets. Facility utilizes a physical sign in/out sheet. Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. A Notice of Site Visit (LIC 9213) was given to Facility Representative, Ana Valadez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with Facility Representative, Ana Valadez.
