What Happened

Follow-up incident visit for child injury. A child was injured on 10/26/23 after falling on loose playground equipment (slide piece from discarded play house that was on the ground). Type A citation issued for immediate health and safety risk. Other playground hazards also observed and required addressing.

Observations

  • Child injured when falling on loose piece of equipment (slide from old Little Tykes play house)
  • Loose equipment was immediate tripping hazard
  • Other playground potential hazards observed and advised for fixing or replacement
  • Director immediately removed loose equipment and disposed of it after incident
