On 5/28/24 @11:30am, Licensing Program Analyst (LPA) Patrick Ma, made an unannounced Case Management inspection regarding a self reported incident made to the Department on 4/16/24. Upon arrival, LPA met with Teacher, Merle Rodriquez prior to Director Alicia Torres retuning to the facility. LPA conducted children and staff interviews, made a confidential names list, and reviewed related documents. Child C1 was injured on the playground with 4 teachers present. Inspection interviews stated at least 2 teachers were less than 8 feet away from C1 and did not observe the incident due to engagement with other children at that moment. There was no visual impediment and teacher were in proper supervising positions on the playground. Multiple teachers attended to C1 once they fell. No deficiencies were cited. Exit interview conducted and report was reviewed with the facility representative Alicia Torres. A notice of site visit was given and must remain posted for 30 days.
