Incident investigation·March 26, 2024

On 3/26/24 at 1:00pm, LPA Patrick Ma made an unannounced visit to conducted case management inspection for the purpose of following up on an incident that was reported on 03/20/2024. C1 was playing with foam blocks on outside playground lawn area, stacking blocks and started to climb on them. C1 was told by Staff S1 to not climb on blocks 2 times. C1 ended up falling off blocks and was injured. C1 was provided comfort and ice. Parents were called. 10:1 ratios were present at time of incident. LPA inspected the playground and foam blocks area. No hazards were observed. Appropriate supervision and ratios were provided. No deficiency cited. Exit interview conducted and report was reviewed with the Director Karisa Riddle. A notice of site visit was given and must remain posted for 30 days.
