What Happened
Follow-up on 6/4/24 fractured elbow incident from play structure fall. Type A deficiency cited for inadequate staff observation of injured child post-incident and concerning play equipment safety issues including inadequate turf cushioning and inappropriate height structure for age group.
Observations
- Child sustained fractured elbow from fall/pull off play structure (6/4/24)
- Staff failed to continuously observe child for injury post-incident despite signs of distress (not eating, wanting to sleep)
- Play structure cushioning inadequate (turf only under 2-5 year structure)
- New tall play structure (6+ feet) labeled for ages 2-5 appears inappropriately sized for age group
