Inspector’s narrative

What the inspector wrote

Based on interviews and documentation it was revealed that child 1 (C1) was injured at approximately 5:20pm and that the responsible party arrived at 5:40pm to pick up C1. At that time the RP was notified of C1s injury. Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED . This report was amended on 2/13/2025 from the original report issued on 10/15/2024. An exit interview was conducted, and this report was reviewed with the Care Programs Manager, Christina Paad , and a copy was provided. Appeal rights were discussed and provided during the exit interview. A Notice of Site visit was given, and Care Programs Manager understands that it must remain posted for 30 days. During interviews it was also disclosed that the staff use the Ring camera to observe when child arrives to start walking out to the car to grab child from transportation. During interviews It was disclosed that child has never walked alone or out of the supervision of the staff to the classroom. LPA observed video recordings on 5 randomly selected days between August 28th and September 20th, including day of question September 6th showing that child was always with supervision from transportation car to classroom. When it comes to allegation that staff do not ensure playground equipment is safe for children, based on interviews and inspection of equipment in question, LPA observed equipment on playground to be safe for children in care if used properly with proper supervision. LPA observed sensory swing to be placed on the grass area of yard, with all four legs secure. LPA observed that legs of swing were not bolted to the ground, however, swing is for indoor or outdoor use and to be used for rocking or sensory input. Lastly, when it comes to allegation that child sustained an injury due to staff not providing adequate supervision, based on interviews staff were present and observed incident. It was also disclosed that staff had spoken with children about the proper ways to use the sensory swing once set up. While on the playground staff were in ratio and positioned near the sensory swing but was not able to prevent the injury. Based on the information obtained during this investigation, it has been determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur. Therefore, the allegations are UNSUBSTANTIATED . An exit interview was conducted, and this report was reviewed with the Care Programs Manager, Christina Paad , and a copy was provided. Appeal rights were discussed and provided during the exit interview. A Notice of Site visit was given, and Care Programs Manager understands that it must remain posted for 30 days.
