What the inspector wrote

On May 22, 2026, at 1:15 PM, Licensing Program Analyst (LPA), William Chancellor, conducted an unannounced Case Management visit to follow up on Unusual Incident Report (UIR) received by Licensing on April 27, 2026. LPA met with Center Director, Roxana Garland who provided a tour of the facility. During the visit, LPA obtained relevant documentation, interviewed three staff and reviewed surveillance. The incident in question occurred on April 23, 2026, between 4:59 and 5:20 PM, involving child 1(C1) being left unsupervised during a playground transition to indoors. Interviews and surveillance verified that C1 was left unsupervised for approximately 14-19 minutes and returned to the after-hour parent meeting by a separate parent arriving. Interviews and record review corroborated that the facility took the necessary steps notifying C1 parents and submitting an unusual incident report in a timely manner. The center implemented renovations for gate locking mechanisms and provided supervision training to staff. Additionally, LPA verified that on the day in question, MAAC program was hosting a parent meeting from 4:30 to 6:00 PM, where parents were present during the time of the incident. To ensure the safety of the children and support of staff during monthly parent meetings, Director Roxana Garland and Area Manager Janet Natividad agreed to implement improved child tracking sign-in sheets, ensure daycare children parents arrive before 5PM on parent meeting dates and supplemental staff assistance is provided when various ages are commingled beyond the limits of the daycare license. Based on the information obtained, and confirmation of the incident occurring after hours, a technical violation will be issued due to an absence of supervision involving a daycare child. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.
