Facility-requested visit·August 3, 2021

On August 3, 2021 at 9:02 a.m. Licensing Program Analyst's (LPA's) Leilani Curtis and Nancy Diaz conducted an unannounced case management inspection with director Maria Teresa Curtis. The purpose of this visit is to inspect the additional rooms that the facility has requested to add to it's license. There were no children present. LPA\342\200\231s toured rooms Lower School Room LS8 and Lower School Art Studio VASC 12 to ensure that the rooms are safe for the care and supervision of children and in compliance with Title 22 regulations. The facility is currently licensed to operate in rooms ECC1, ECC2, ECC3, ECC4 and ECC5 located in the Lower School. The facility is also using the library, music room and gymnasium for additional activities. The facility has requested to add the following rooms to the license: Lower School Room LS8 and Lower School Art Studio VASC12. The fire clearance for the additional rooms was granted on 7/13/21. The licensed capacity will remain unchanged at 74 children, ages 2 through Kindergarten. The following rooms were measured: Lower School Room LS8 has 572 sq. ft. of indoor space. Lower School Art Studio VASC 12 has 1022 sq. ft. of indoor space. The total indoor space for these two classrooms is 1594 sq. ft. and will accommodate 45 preschool children. No deficiencies are cited. The addition of these two rooms for childcare is granted and effective today's date, August 3, 2021. The licensed rooms are as follows: ECC1, ECC2, ECC3, ECC4 and ECC5. The library, music room, gymnasium, Lower School LS8 and Lower School Art Studio VASC 12 will be used for additional activities. An updated license will be sent for posting. An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director\342\200\231s signature on this form acknowledges receipt of these rights.
