Licensing Program Analyst (LPA), Keely Messerschmidt conducted an annual inspection as part of a compliance review. A tour of the inside and outside of the facility was granted and the following was observed and/or noted: \302\267 The following items were posted and updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent\342\200\231s Rights Poster (PUB393) - Personal Rights (LIC613A) - Child Car Seat Law - Menu \302\267 The facility is operating within the limits as stated on the license. \302\267 Ratios are being met during this inspection, there were 13 children with 2 staff members \302\267 Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards \302\267 There are no weapons present at the facility as stated by Director \302\267 There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Drinking water is provided in the indoor activity space and in the outdoor activity space by filtered water \302\267 No medications at this time \302\267 Hazardous items are stored where inaccessible to children which include disinfectants, cleaning solutions and other items that are dangerous \302\267 Poisons and toxins are locked and inaccessible to children \302\267 All floors were observed to be safe and clean. \302\267 Bathrooms were observed to be safe, sanitary and in operating condition \302\267 Playgrounds are enclosed by appropriate fences and free of hazards. During inspection, broken toys were observed and picked up. \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition \302\267 Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin \302\267 Food is stored appropriately and protected from contamination \302\267 All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair \302\267 Sign in/Sign out record was reviewed and meets regulation requirements \302\267 Disaster drills are conducted at least every six months \342\200\223 last drill was conducted on 11/20/24 A review of staff and children's records were conducted as part of this evaluation. \302\267 Children\342\200\231s records were found to be complete during this inspection. C4 missing TB and C6 missing Personal Rights form in file. \302\267 Staff record review indicates that all staff present do not meet minimum qualifications for the position for which they were hired. See LIC809D page. \302\267 No staff member present with current Pediatric CPR/First Aid. See LIC809D page. \302\267 Director is pending completion of his Health and Safety Training \302\267 A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. LPA informed Director that he needs to associate his fingerprints to facility license. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 , requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Director was unable to provide a copy of completed lead testing during inspection. See LIC809D page. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. On-line Licensing forms & regulations for a Child Care Center can be obtained on the Department\342\200\231s website: www.ccld.ca.gov . Additionally, there is a link to \342\200\234Receive Important Updates\342\200\235 located on the right side of the page, immediately above Quick Links. One can add their email address and choose which program(s) they wish to receive Provider Information Notices (PIN) for. The Duty Officer is available to answer questions Monday \342\200\223 Friday; 8:00am to 5:00pm at: 951-782-4200. The director was asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made or file copy is more than 2 years old) 4. LIC 309 Administrative Organization (only if changes have been made or file copy is more than 2 years old) 5. LIC 308 Designation of Administrative Responsibility (only if changes have been made & current designation is on file) See LIC809-D for cited deficiencies. During the exit interview, the Director Timothy Bagby, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. LPA gave copy of past due notice for annual fees to Director. An exit interview was conducted, and this report was reviewed with the Director Timothy Bagby. Appeal rights were discussed and provided during the exit interview.
