On May 5, 2022 at 9:56 am, Licensing Program Analyst (LPA) Cindy Hamilton conducted an unannounced case management inspection at Children’s Paradise to follow-up on an Unusual Incident Report (UIR) submitted by the facility on April 21, 2022. During the inspection, LPA toured the facility, took census, and met with Site Director Natalie Swisse to discuss the reported incident. LPA also interview staff and reviewed pertinent files. The child involved in the incident was not interviewed by the LPA due to child being an infant and unable to verbalize. Based on the information obtained there appeared to be no violations of Title 22 Regulations therefore no citations were issued. An exit interview was held with Director Natalie Swisse. A copy of this report was issued, along with a Notice of Site visit. This report shall be public record for three years.
