Inspector’s narrative

What the inspector wrote

On 1/11/24, an unannounced Case Management-Deficiency inspection was conducted by Licensing Program Analyst (LPA) Saraliz Velando. The purpose of the inspection is to follow up on an unreported incident. Upon arrival, LPA met with licensee, Gabriela Barbosa De Araujo. LPA conducted a tour of the home and observed 6 children in care. During the incident on 10/11/23, there was a call placed to 911 and an adult resident was arrested. Licensee failed to report the unusual incident that occurred during daycare hours while there were children in care, to the department, as per regulations. Based on the information received by the department, it appears that the licensee did not comply with Title 22 regulation section. Type B Violation was cited. Refer to the next page LIC 809-D for deficiency citation. Facility was provided a copy of the appeal rights. The exit interview was conducted, and the report was reviewed with licensee, Gabriela Barbosa De Araujo. A notice of site visit was posted and shall remain for 30 days.
