What the inspector wrote
On 6/27/22 at 2:45 PM Licensing Program Analyst (LPA), Adrian Mangina, conducted an announced Pre-Licensing Change of Location Inspection for a large family child care home with the Licensee, Shaha Alawad. Also in the home was Licensee\342\200\231s mother Sabah Alawad, adult brother Anwar Alawad and father Faisal Alawad, and 2 minor sibling one of whom who assisted with translation. Licensee owns the home and has provided proof of control of property. The 4 bedroom, 2 bath, three story home was toured and inspected to ensure an environment safe for the care and supervision of children. The 2A10BC fire extinguisher, smoke alarm and carbon monoxide detector, all located in the kitchen meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There is a hot tub in the back yard which does not have a locking cover that can hold the weight of an adult. Licensee states that the hot tub will not be used for children in care. LPA advised licensee needs to secure the hot tub per bodies of water regulations before License will be issued. Licensee will use the neighborhood park for outdoor activities. Applicant understands visual supervision is to be maintained at all times during outdoor activities. Applicant states that there are no weapons in the home and has certified on the application they have sufficient financial resources to sustain the license. Licensee owns home and has provided a copy of the mortgage payment receipt as proof of control of property. Fire Clearance was granted 5/10/22.
First Aid and CPR expire 9/2023 and assistant\342\200\231s First Aid and CPR expires 1/24/2023. Licensee\342\200\231s preventative health practices course was completed on 06/16/2019. Applicant and assistant are exempt from Mandated Reporter Training AB 1207, as primary language is Arabic. Staff immunization requirements per SB792 were met and TB requirements were met. Required documents were not posted and must be before Licensure. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children,

and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Licensee will be using the following rooms for childcare: Living room, dining area, kitchen and ground level family room. The following areas will be off limits: entire top floor, attached garage and front and back yards. Off-limit areas are made inaccessible through use of gates ,locks, and safety latches. The Applicant has safe, age appropriate toys and equipment available for children.

The new provider packet was reviewed with the applicant including information on ratios and capacity, child abuse reporting, children\342\200\231s records, immunizations, adults living or working in the home, car seat law, shaken baby syndrome, SIDS, safe sleep practices, effects of lead poisoning, and the YMCA Resource Center. Applicant was reminded that corporal punishment, smoking, walkers, exersaucers, jumpers, and bouncy seats are not allowed in day care. and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with licensee, the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

Applicant owns home and, when assistant is present may care for up to 14 children, including any children living in the home under ten years old as long as there are two school aged children enrolled (with one enrolled in kindergarten).

The following corrections are needed prior to the issuance of the license:
1) Install locking cover that holds the weight of and adult on hot tub in back yard.
2) Licensee must post all required documents
3) Licensee must install second gate on bottom of stairway that leads to family room.
Applicant understands that corrections must be submitted to the Department within 30 days, no later than 7/26/22 or the application may be denied.
