What the inspector wrote
On 8/1/23 at 8:30 AM, Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced Annual inspection with the Licensee. Upon arrival, LPA met with Licensee, Ghiahib Al Dujaili. The one-story three bedroom three-bathroom single story home was toured and inspected to ensure an environment safe for the care and supervision of children. Also present in the home were Licensee\342\200\231s husband Haitham Al Shakarchi and one minor child. There were no children in care. Proper supervision and ratios were observed. The 2A10BC fire extinguisher, carbon monoxide detector, and smoke detector located in kitchen meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water on the property. Licensee states that there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Licensee\342\200\231s First Aid and CPR certifications expire on 5/2025. Licensee meets immunization requirements. Mandated Reporter Training was waived due to language. Licensee maintains emergency records for children. LPA reviewed 5 child files which were complete. Licensee had current child roster. Last disaster drills were conducted 3/31/23.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include: kitchen, dining room, living room, bathroom 2 and bedroom 1. Off limits areas

include: bedroom 2, bedroom 3, bathroom 1, bathroom 3, and living room 2 and are inaccessible through use of doorknob covers. The fireplace in living room is screened. There is a working phone at the facility. The licensee has sufficient age appropriate, safe, toys and equipment available. The home has a fully fenced front yard available for outdoor activities. Visual supervision is required all times when children in care are outside.
Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children\342\200\231s records according to regulation, post all required forms, ensure that all adults living or working in the home have criminal background clearances associated to the facility, corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. LPA discussed the safe sleep regulations with LIcensee and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA reminded Licensee to follow safe sleep guidelines when enrolling children 2 years old and under.
On this date, 8/1/23 the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered
sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Licensee was informed of the
MyChildCarePlan.org
site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The Licensee owns own home and has previously provided proof of control of property. The Licensee is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care for up to 14 children with a Large Family Child Care License. Entrance Checklist was provided to the applicant.

Exit interview conducted and report was reviewed with the Licensee, Ghiahib Al Dujaili. A notice of site visit was given to Licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
