What the inspector wrote

On June 26, 2025, at 8:30 a.m., Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding the allegations listed above. During the visit, LPA Sutherland met with the Licensee, Briksam Gad. Also present in the home were the Licensee's sister, Jazmine Gad, and the Licensee's parents, Fadila Eishanawani and Medhat Gad. There were no daycare children present. The Department conducted a thorough investigation into the allegations. LPA toured and inspected the home on 4/22/25. LPA conducted interviews with current parents, family members, potential witnesses and Licensee. Licensee confirmed that children are always supervised. Licensee understands that she must only use highchairs and other swings as intended for by the manufacturer. The main childcare area is located upstairs in the home. The children typically have snack time either in the upstairs area or outside in the backyard, depending on the day's activities. Highchairs are utilized in both areas to facilitate snack time and are appropriately maintained to ensure the safety of the children during use. Based on staff, parents’ interviews, the above-referenced allegations could not be conclusively proven or disproven. Therefore, they are considered Unsubstantiated. A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. No deficiencies are cited. An exit interview was conducted with the Licensee. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) was provided along with the report (LIC9099) to the Licensee. LPA Sutherland observed Notice of Site Visit being posted. Notice of Site Visit must be posted for 30 days.
