What the inspector wrote

On 05/22/2025 at 2:00pm Licensing Program Analyst (LPA) Veronica Torres Gonzalez conducted an unannounced visit for an unrelated matter. LPA was greeted at the door by minor son, then greeted by licensee Parwin Kohistani. Present were 3 adults and 3 childcare children. At 2:30 PM daughter Hila, Kohistani arrived with minor sister. Hila Kohistani helped with translation. Hours of operation are from 7am to 10pm. At 3:54PM two childcare children were picked up by parents. At 5:30 3 more children arrived at the facility. LPA Interview licensee, obtained facility roster, reviewed children\342\200\231s records. 6 out of the 37 children files are missing immunization cards, (C1 - C6) and 11 out of the 37 total children enrolled files are missing all documents (C7-C17) and roster provided was incomplete. Exit interview conducted and report was reviewed with licensee Parwin, Kohistani. 809 report was printed and provided to licensee Parwin, Kohistani .
