What the inspector wrote

On 07/16/2025 @ 11:00 AM, Licensing Program Analyst (LPA) Veronica Torres Gonzalez conducted an unannounced case management for the purpose of delivering findings. LPA met by assistant and daughter Arzo Kohistani who assisted with translation. LPA was later greeted by licensee Parwin Kohistani. LPA toured the home with licensee Parwin Kohistani. Observed present today were 5 adults, 3 Children including licensee's daughter and 1 infant. All adults have been fingerprinted and associated with the facility. Children\342\200\231s records were reviewed during inspection and found no infant sleep logs for children in care. LPA explained the importance of the sleep logs and reviewed regulation with licensee to confirm understanding. The following deficiencies were cited please refer to 809-D.
