What the inspector wrote

On 08/01/2025 Licensing Program Licensing (LPA's) Veronica Torres Gonzalez and Nancy Diaz conducted an unannounced case management for the purpose of delivering a citation on LIC 9099 that was omitted on 07/16/25. Present was Licensee, licensee's oldest daughter (Arzo Kohistani), licensee son (Samim Kohistani) and licensee's Husband (Habibullah Kohistani.) Exit interview was conducted with licensee Parwin, Kohistani. LPA reviewed report and provided a copy. Notice of site visit was given and must be posted for 30 days.
