What the inspector wrote

On 9/19/25 at 9:40am Licensing Program Analyst (LPA) Sharon Mendez conducted an unannounced inspection with the Licensee Rand Alzahawi. LPA identified self, disclosed the purpose of the inspection annual visit with a change of capacity and was granted entry into the facility by the Licensee. Present in the home was only the Licensee and no day care children were present. The 3 story home was toured and inspected to ensure an environment safe for the care and supervision of children. A fire clearance was approved on 09/12/2025. Licensee accompanied LPA inside and out of the facility during this inspection. Licensee has provided adequate space for the children to eat, sleep and play within the home. Applicant will be using part of the second floor of the home which include Living Room, Dining Room, Kitchen, Bedroom 3, and Bathroom 2 for childcare. Off limit areas are bedroom 2, and bottom floors known as first floor that includes bedroom 1 and bathroom 1, and the third floor upstairs which has the master Bedroom 4 with a bathroom inside, the garage, and backyard and are inaccessible by use of door locks and the stairs are barricaded. The licensee has sufficient toys and equipment available. Licensee will use nearby park for outdoor activities, licensee understands there must be direct supervision while at the park with children. The fire extinguisher located in the dining room, smoke detector, and carbon monoxide detector located in the hall meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. The fireplace is screened. There are no bodies of water. Licensee states that there are no weapons in the home. First Aid and CPR certifications expire on 12/25. Licensee has required immunizations. Licensee completed Mandated Reporter Training on 12/23/24. Children\342\200\231s and Staff records were reviewed and found to be in order. Continued on page 2... Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee is hereby reminded of the following: Report suspected child abuse and neglect, maintain children\342\200\231s records according to regulation, post all required forms, corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee states is registered to receive Provider Information Notices (PINs). LPA discussed and provided Licensee with the following: child care advocates-email address: childcareadvocatesprogram@dss.ca.gov . During the exit interview, it was confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS on 9/19/25. No deficiencies cited Upon final review Licensee will be granted a large license with capacity to take care up to 14 children. Exit interview conducted and report was reviewed with the licensee Rand Alzahawi. A notice of site visit was given and must remain posted for 30 days.
