Pre-licensing visit·January 15, 2025

On 01/15/2025 at 2:00 P.M, Licensing Program Analyst (LPA) Edleen Montesa conducted an announced Pre-licensing inspection due to a Change of Location with the licensee, Ibrahim Almohammad, and Fatima Almohammad, licensee\342\200\231s adult daughter was present, to assist in language translation, as licensee\342\200\231s primary language is Arabic, with given permission from the licensee. LPA was granted entry after showing badge, identifying self, and stating the reason for the visit. The three bedroom, two bathroom, apartment located in the first floor was toured and inspected to ensure an environment safe for the care and supervision of children. Licensee will be using the living room, kitchen, family room, bathroom near hallway, and backyard for day care operations. Off limit areas are master bedroom, Bedroom 2, Bedroom 3, and master bathroom and are inaccessible by use of child safety door knob covers, locks, safety latches. The licensee has play materials, and equipment available in the home. Licensee will utilize backyard for outdoor play, area is fenced and is free of hazards, applicant has been advised to ensure direct supervision of daycare children at all times. LPA observed a locked side gate/screen door towards the farther end side of the house near Bedroom 3. LPA observed that the area is used as storage and is locked. Licensee stated that the area is used by neighbor\342\200\231s for storage space and is locked at all times and he does not have access. The fire extinguisher is located in the kitchen, carbon monoxide detector is located in the kitchen, and smoke detector is located in the hallway, meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There is a gated community pool in the apartment complex, and there is no direct access from the apartment unit. The fenced community pool can only be accessed with the use of an assigned key. Licensee states that there are no weapons in the home. Licensee states that they have sufficient financial resources to sustain the license. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The licensee provided proof of control of property. Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the licensee confirms was provided to the property owner/landlord. The licensee obtained a signed Property Owner/Landlord Consent form (LIC 9149). CPR and First Aid certification expires on 10/21/2025 and Preventative Health Practices course which includes nutrition and lead training, was completed on 10/15/2023. Licensee is exempt from Mandated Reporter Training requirement due to English is not his/her primary language). Immunization requirements per SB792 were met. Required documents have been posted. LPA reviewed with licensee the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the licensee. The New Provider packet was reviewed with the applicant including information on ratios and capacity, child abuse reporting, unusual incident reporting, children\342\200\231s records, immunizations, personnel records, car seat law, Shaken Baby Syndrome, Sudden Infant Death Syndrome, effects of lead poisoning, required facility postings, and community resources. Licensee was reminded that corporal punishment, smoking, walkers, exersaucers, jumpers, and bouncy seats are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, 01/15/2023 the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following corrections are needed prior to the issuance of the license: 1. Proof of tuberculosis clearance for Fatima Almohammad. Licensee understands that corrections must be submitted to the Department within 30 days or the application may be denied. Licensee stated that they may apply for a capacity increase. LPA provided information regarding the application of a capacity increase and advised the applicant of the requirements. Licensee agreed to comply with all regulations and laws governing family child-care homes. Exit interview conducted and report was reviewed with the licensee, Ibrahim Almohammad. A notice of site visit was given to applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
