What the inspector wrote

On 05/08/2025 1:15PM , Licensing Program Analyst, Hanna Lucas, made an unannounced visit to evaluate the facility for a requested increase in capacity. LPA was greeted at the door by Licensee\342\200\231s wife, Fawzah Abdulhaseeb, and his adult daughter, Fatima Almohammad. Licensee, Ibrahim Almohammad, greeted LPA once inside the apartment. The request was received on 02/11/2025 and the Fire Clearance was granted on 04/22/2025. LPA inspected the facility and there were zero children in care. LPA briefly reviewed record keeping, and maintaining the Children\342\200\231s Roaster. Licensee is exempt from Mandated Reporter because their first language is Arabic and First Aid/CPR Certificate until October 2025. Capacity limitations were discussed, and a handout was provided. Licensee understands that if an assistant is not present, the facility must operate at small family capacity. LPA reminded Licensee that an assistant must have proof of a negative TB test, a valid Mandated Reporter Certificate, proof of Immunizations, be fingerprint cleared and associated to the facility. If the Licensee leaves the assistant alone, they must have a valid CPR/First Aid Certificate. Licensee plans to have his adult daughter, Fatima Almohammad, as an assistant. Licensee understands he must submit all required documentation to the Department prior to the assistant helping watch over the children. LPA explained to the Licensee, that he can only be gone for 20% of the time or the facility must close all together. Upon a final file review, the request for increase in capacity will be approved and an updated license will be mailed to the Licensee for posting.
