Pre-licensing visit·March 6, 2025

On 03/06/2025 @ 01:00 p.m., LPAs Mahjoba Mohsini and Evelyn Reyes conducted an announced comprehensive visit for the purpose of a pre-licensing inspection. LPAs were greeted at the door by the Applicant, Gulbahar Hemrayeva and also present was her assistant (daughter, Zubeyda Mullyk). and LPAs were granted entry after identifying self and disclosing the purpose of the visit. Entrance Checklist was provided to the applicant. Because the Applicant, rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant has obtained a signed Property Owner/Landlord Consent form (LIC 9149). Once licensed, they can operate with maximum capacity of 8 children. LPAs confirmed with the Applicant that all adults living in the home have been fingerprint cleared and associated with the facility and have met all the immunization requirements. Applicant has provided evidence of the completion of the required 16-hr. Health and Safety training to include a Pediatric First Aid/CPR certificate with ABC, valid through 02/2027 and Mandated Reporter Training valid through 1/2027. Assistant's (Zubeyda Mullyk) Pediatric First Aid/CPR certificate with ABC, valid through 03/2027 and Mandated Reporter Training valid through 1/2027 A tour of the one-story home (4 bedrooms, 2 bathrooms, garage and fenced back yard and fenced front yard) was conducted. Childcare areas are: Bedroom #2 & # 4, living/dining area, kitchen, bathroom #2), and fenced backyard. Off limit areas include: all bedroom #1 and Bedroom #3 (office) and bathroom #1 and have been made inaccessible with doorknob covers. Applicant stated there are no weapons in the home. There are no bodies of water on the facility property. There is a fireplace in the living room and has been barricaded. Poisons, detergents, cleaning solutions, medications are made inaccessible through use of latches and locks. LPAs tested the smoke alarm and carbon monoxide detectors located in the hallway and they were operational. The fire extinguisher is fully charged (near the entrance door) and meets regulations. There is a working telephone, and Parent\342\200\231s Rights form is posted. The home is kept clean and orderly with adequate heating and ventilation for safety and comfort. Outdoor play will take place at the fenced back yard. There is a sufficient amount of age-appropriate toys, books and games for children, in good condition. LPAs reviewed with Applicant Form LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. LPAs discussed the safe sleep regulations with Applicant and the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. www.meganslaw.ca.gov Exit interview conducted and report was reviewed with the Applicant. During the exit interview, the Applicant, confirmed that there are no Registered Sex Offenders living in the facility. . No corrections are required. Upon final file review and approval of the application, a license for 8 children will be sent for posting.
