What the inspector wrote

THIS IS AN AMENDED VERSION OF AN ORIGINAL REPORT DATED 4/11/25 On 04/11/2025 @ 12:00 p.m., LPA Mahjoba Mohsini conducted an unannounced case management visit requested by Licensee for the purpose of capacity increase. LPA was greeted at the door by the Licensee (Gulbahar Hemrayeva) and also present was her assistant (daughter, Zubeyda Mullyk). LPA was granted entry after identifying self and disclosing the purpose of the visit. Entrance Checklist was provided to the applicant. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care are: tour of the one-story home (4 bedrooms, 2 bathrooms, garage and fenced back yard and fenced front yard) was conducted. Childcare areas are: Bedroom #2 & # 4, living/dining area, kitchen, bathroom #2), and fenced backyard. Off limit areas include: bedroom #1 and Bedroom #3 (office) and bathroom #1 and have been made inaccessible with doorknob covers. Applicant stated there are no weapons in the home. There are no bodies of water on the facility property. There is a fireplace in the living room and has been barricaded. Poisons, detergents, cleaning solutions, medications are made inaccessible through use of latches and locks. The licensee has sufficient age appropriate toys and equipment available. The Licensee has developed and follows a structured daily schedule that incorporates age-appropriate play and learning activities. The facility has designated areas to support theses activities. THIS IS AN AMENDED VERSION OF AN ORIGINAL REPORT DATED 4/11/25. Meals are prepared on-site by the licensee and provided to the children during scheduled meal times. A weekly menu is maintained for parental review and. The menu includes a variety of meals and snacks that meet the nutritional requirements outlined in Title 22 regulations. LPAs confirmed with the Applicant that all adults living in the home have been fingerprint cleared and associated with the facility and have met all the immunization requirements. Applicant has provided evidence of the completion of the required 16-hr. Health and Safety training to include a Pediatric First Aid/CPR certificate with ABC, valid through 02/2027 and Mandated Reporter Training valid through 1/2027. Assistant's (Zubeyda Mullyk) Pediatric First Aid/CPR certificate with ABC, valid through 03/2027 and Mandated Reporter Training valid through 1/2027. LPAs tested the smoke alarm and carbon monoxide detectors located in the hallway and they were operational. The fire extinguisher is fully charged (near the entrance door) and meets regulations. There is a working telephone, and Parent\342\200\231s Rights form is posted. The home is kept clean and orderly with adequate heating and ventilation for safety and comfort. Outdoor play will take place at the fenced back yard. There is a sufficient amount of age-appropriate toys, books and games for children, in good condition. LPA discussed the safe sleep regulations with Applicant and the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated . Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Select \342\200\234Receive Important Updates\342\200\235 then put the email address in and choose which program(s) you would like to subscribe to and select \342\200\234subscribe.\342\200\235 On this date, 04/07/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. www.meganslaw.ca.gov Exit interview conducted and report was reviewed with the Applicant. During the exit interview, the Applicant, Gulbahar Hermrayeva confirmed that there are no Registered Sex Offenders living in the facility. . No corrections are required. Upon final file review and approval of the application, a license for 14 children will be sent for posting.
