Pre-licensing visit·October 2, 2025

On 10/2/2025 @ 8:45AM, a follow-up prelicensing inspection was conducted with applicant Soheila Golabi. Her spouse, Ali Hami was also present. The purpose of this follow-up prelicensing inspection is in reference to observing proof that applicant resides in the home. Mrs. Golabi has completed her move and lives in the home full-time. Mrs. Golabi has proven that she has established residency. A license for a large family child care is approved effective 10/2/2025. Maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home: 12 children, with no more than four infants or 14 children with no more than 3 infants and at least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age. Exit interview was conducted with Mrs. Golabi. LPA reviewed and provided a copy of this report.
