What the inspector wrote

On 09/28/2022 at 10am, Licensing Program Analyst (LPA) Samantha Clenista conducted an unannounced case management site inspection. Upon arrival, LPA met with Center Director, Angie Triana. This facility requested an increase of capacity from having 157 children to 168 children. The program is currently licensed to care for up to 120 preschoolers (ages 3-K) and 37 toddlers (ages 18-36 months). Facility has requested to increase their toddlers to 48 children. There is no change for the preschool capacity or licensed areas. Fire clearance was granted on 08/11/2022. LPA conducted a tour of the preschool and toddler program areas. LPA observed a total of 134 children with a total of 22 staff. Children were observed either indoors participating in circle time/art activities or outdoors playing. Appropriate ratio, capacity and supervision was observed during inspection. All classrooms and outdoor play areas were inspected. LPA observed the indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Outdoor play area is fenced, has wood chips, sand and foam mats for cushioning and umbrellas/trees used for shade, age appropriate play equipment, and sufficient cushioning under the climbing structure. There is a working carbon monoxide detector located in the facility's main hallway. The classrooms and outdoor playground were previously measured on 11/14/2019. All toddler classrooms (room 102, 103, 104 and 105) measured at a total of 1,777.43 sq. ft., sufficient for 50 children. Outdoor playground measured at a total of 2,779.38 sq. ft., sufficient for 37 children. Facility is requesting for 48 toddlers. A playground waiver must be submitted and approved prior to licensure. Drinking water is available in all of the classrooms and outdoors via children's water bottles that are brought from home and Brita filtered. Last fire drill was conducted and documented on 09/02/2022. Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed the updated LIC311A with the facility representative today and reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children. LPA obtained a playground waiver request from Director at conclusion of visit. A license for 120 children (ages 3-K) in rooms 106, 107, 108, 109 and 110 with 48 toddlers (18-36 months) in rooms 102, 103, 104 and 105 may be granted upon receiving an approved playground waiver and after a final file review. Exit interview was conducted with Director. LPA will email Director all Provider Information Notice's regarding Lead Testing. LPA briefly reviewed the information and Director stated her understanding of the information and what is needed to be done by January 1, 2023. This report shall be kept available at the facility for public review for 3 years. Notice of Site Visit was provided and was observed posted. Director is aware that the NOS form shall be posted for 30 consecutive days.
